CONNECT Summit County is proud to offer a Mental Health Financial Assistance program for individuals living and/or working within Summit County limits. We are available to answer questions at 435-776-4357 (English) or 435-655-1230 (Spanish).
Financial Assistance Application

With support from donors and community members, we help you overcome the cost of therapy and provide you with access to local mental-health services. If financial hardships have been stopping you from receiving the care you need, this may be a good opportunity to put yourself first.
This program is meant to complement the community’s various resources, such as local Employee Assistance Programs, Financial Assistance Programs offered by local providers, as well as health insurance and Medicare/Medicaid.

- If more than one family member is interested in applying for financial assistance, please complete one form per person.
- Once you submit your application, you will receive a text/email/call to let you know we have received your application.
PROGRAM GUIDELINES
- The application must be submitted and approved prior to any sessions taking place.
- Each client is eligible for 12 sessions a year. Sessions are approved at an $85 rate.
- Clients may not re-apply within the same year.
* CONNECT will evaluate special need requests on a case-by-case basis. - Other options will be evaluated and provided to clients should it be necessary.
- CONNECT Summit County has a limit on its ability to subsidize and reduce fees.

THE MODEL
Building Hope in Summit County, Colorado has been running mental health scholarships, or vouchers, in their community for several years. Watch the video to learn about the successes of the program from Ravi Jaishankar, Building Hope’s scholarship coordinator, and Cindi Harris-Krane, a Summit County, Colorado mental health provider. CONNECT Summit County will be launching their own version of mental health scholarships in 2022.
Provider Roundtables hosted by CONNECT Summit County give opportunities for all providers of mental health services to come together and connect, give and receive support, share, encourage, and learn from one another.
